General Support: Making a specific Theme.

I recently downloaded the Element theme, however, What I'm after is a theme that works ina similar principle, but instead of thaving the small thums at the top, having them run along the left of the page, with a scrollbar to contain the list going down too far.

If I design what I'm after, Is anyone able to advise me on how to go about it? I'm willing to release the theme for free after.
